As it may not be possible for everyone to visit St James or St Boniface to place their Holy Souls List by the altar we have put together the following form which you can fill in which will be emailed to the parish office.

Once received we will place this alongside the other lists on the altar.

Alternatively, if you prefer you can download the form to fill in by hand and either email, send to the parish or drop off to the parish office.
